How to Use an Interactive PDF Form

Download the interactive PDF file:
PC users: right click on the link and choose "Save Target As"
Mac users: Option+ Click on the link and choose "Save"
Save it to your computer desktop or in the folder of your choice.

Open the interactive PDF file: You need the latest version of the free program Acrobat Reader to access the Typewriter Tool. If you already have Acrobat Reader, you may need to download a newer version. You can download it at http://www.adobe.com/products/acrobat/readstep2.html.

Fill out the form: Click on the Typewriter button at the top of the document and follow the instructions found there.

Save the form: You can open, enter or change information, save, and print the form multiple times.

Send the form: You may want to email the form as an attachment. Warning! If your form contains credit card information, DO NOT send it by email. Email is not secure and your credit card information could be stolen. Send your form by mail or fax. Or use the secure, online forms found on other parts of our website.
